PROOF OF SOCIAL SECURITY NUMBER:
Social Security Card, W-2 form OR a pay check stub with your social security number.
PROOF OF RESIDENTIAL ADDRESS (two or more originals and your license does not count as one):
Deed, mortgage, monthly mortgage statement, mortgage payment booklet or residential rental/lease agreement OR a Florida Voter Registration Card OR Florida Vehicle Registration or Title OR a Florida Boat Registration or Title (if living on a boat/houseboat) OR a utility hook up or work order dated within 60 days of the application OR Medical or health card with address listed OR a selective service card.
